In the cargo operation process, the use of safety equipment is very important because it is used to protect yourself from accidents when we work on decks safely and safely. The problems discussed in this thesis are not optimal use of safety equipment during the cargo operation process The research method used is Fishbone, this method is used to determine the factors of failure of a system in the form of man, equipment, rule, enviroment. Furthermore, the factors that have been found will be sought roots and solutions using the Urgency, Seriousness, Growth (USG) method to find out prevention efforts in this problem. The data sources used are interview research and documentation. The results of this study found several factors that resulted in not optimal preparation of safety equipment. Damage to safety equipment. Non-international standard equipment. No strict company regulations. Lack of supervision from officers. The lack of awareness of the crew of the ship will be dangerous. Habits of a bad work environment. The researcher concluded that the efforts that can be made include increasing the crew crew discipline in the use of personal safety equipment. Familiarization of the crew of the ship. Provide motivation to the crew. Provide more supervision to the crew for those who violate. The conclusion of this thesis is the lack of awareness in the preparation of safety equipment properly and correctly. So it is recommended that cargo operation activities are always monitored by Officer watch so that the crew can prepare safety equipment properly according to the procedures. Dalam proses cargo operation, penggunaan alat keselamatan sangatlah penting karena digunakan untuk melindungi diri sendiri dari kecelakaan saat kita bekerja di deck dengan aman dan safety. Permasalahan yang dibahas pada skripsi ini adalah tidak optimalnya penggunaan alat keselamatan pada saat proses cargo operation Metode peneitian yang digunakan adalah Fishbone, metode ini digunakan untuk menentukan faktor dari kegagalan suatu sistem yang berupa man, equipment, rule, enviroment. Selanjutnya faktor yang sudah ditemukan akan dicari akar dan penyelesainnya menggunakan metode Urgency, Seriousness, Growth (USG) untuk mengetahui upaya pencegahan dalam permasalahan ini. Adapun sumber data yang digunakan yaitu riset wawancara dan dokumentasi. Hasil penelitian ini menemukan beberapa faktor yang mengakibatkan tidak optimalnya persiapan alat keselamatan. Kerusakan pada perlengkapan alat keselamatan. Perlengkapan tidak berstandar international. Tidak tegasnya peraturan perusahaan. Kurangnya pengawasan dari perwira. Kurangnya kesadaran kru kapal akan bahaya. Kebiasaan lingkungan kerja yang buruk. Peneliti menyimpulkan upaya-upaya yang dapat dilakukan antara lain yaitu dengan meningkatkan disiplin kru kapal dalam penggunaan alat keselamatan diri. Familirisasi terhadap kru kapal. Memberikan motivasi kepada kru. Memberikan pengawasan lebih kepada kru bagi yang melanggar. Simpulan dari skripsi ini adalah Kurangnya kesadaran dalam persiapan alat keselamatan dengan baik dan benar. Maka disarankan agar kegiatan cargo operation selalu di awasi oleh Mualim jaga agar kru kapal dapat mempersiapkan alat keselamatan dengan benar sesuai prosedur yang ada.
