Abstract

Sungai Gerong", Diploma IV Program, Nautika, Semarang Shipping Science Polytechnic, Advisor I: Capt. Eko Murdiyanto,M.Pd,M.Mar and Advisor II: Abdi Seno,M.Sc. In the cargo watchkeeping if the vessel is carrying out avtur discharging, preparation can be made to support the smooth operation of discharging activities so that there is no negligence in the implementation of guard services that can cause damage, accidents of environmental pollution, especially the port environment. For guard officers in carrying out their duty to guard, they must continue to carry out careful inspections or supervision during the loading and discharging process to realize safe and controlled discharging activities. The research method used descriptive and qualitative in this paper and then used fishbone analysis method and fault tree analysis as a method to determine what factors exist in the problem. The formulation of the problem from this research is why there was negligence in carrying out guard duties at the semampir timur port of Surabaya during the avtur discharging on the MT. Sungai Gerong, what is impact if there is negligence in carrying out guard duties at the port during the avtur discharging on MT. Sungai Gerong, and what efforts to overcome so that there is no negligence at the time of guarding. Based on the result of this study, it was concluded that the negligence of the service of avtur guarding was that there was an oil spill on the deck of the MT. Sungai Gerong, there was sea pollution of the port area. The impact of negligence on guarding and discharging aviation fuel is the implementation of a less than maximum guard service, endangering the safety of the human life. Effort to overcome the failure of the avtur discharging service at the Semampir Timur port of Surabaya to take place are to apply the avtur discharging procedures in accordance with the SOP, applying the principles of occupational health and safety for the guard officers. Eko Murdiyanto,M.Pd,M.Mar dan Pembimbing II: Abdi Seno,M.Si Dalam rangka pelaksanaan dinas jaga dilakukan apabila kapal sedang melaksanakan bongkar maka diperlukan persiapan yang dapat menunjang kelancaran dari kegiatan bongkar agar tidak terjadi kelalaian pelaksanaan dinas jaga yang dapat menyebabkan kerusakan, kecelakaan, maupun pencemaran lingkungan, terutama lingkungan pelabuhan. Bagi petugas jaga dalam melaksanakan tugas jaga hrus terus menerus melakukan pemeriksaan atau pengawasan secara seksama selama pelaksanaan bongkar berlangsung guna mewujudkan kegiatan bongkar yang aman dan terkendali. Metode penelitian yang digunakan dalam skripsi ini adalah desktiptif dan kualitatif serta menggunakan Fishbone Analysis dan Fault Tree Analysis sebagai metode untuk menentukan faktor apa saja yang ada pada permasalahan. Adapun rumusan masalah dari penelitian ini adalah mengapa terjadi kelalaian pelaksanaan tugas jaga di pelabuhan Semampir Timur Surabaya pada saat bongkar avtur di kapal MT. Sungai Gerong, bagaimana dampaknya jika terjadi kelalaian dalam menjalankan tugas jaga di pelabuhan pada saat bongkar avtur di MT. Sungai Gerong, dan apa upaya untuk menanggulangi agar tidak terjadi kelalaian pada saat dinas jaga. Berdasarkan hasil penelitian ini disimpulkan bahwa kelalaian pelaksanaan dinas jaga avtur adalah terdapat tumpahan minyak di dek kapal MT.Sungai Gerong, Terjadi pencemaran laut karena jatuhnya minyak ke laut daerah pelabuhan. Dampak dari kelalaian dinas jaga bongkar avtur adalah pelaksanaan dinas jaga yang kurang maksimal, membahayakan keselamatan jiwa manusia. Upaya untuk menanggulangi agar tidak terjadinya kelalaian pelaksanaan dinas jaga bongkar avtur di pelabuhan Semampir Timur Surabaya adalah menerapkan prosedur bongkar avtur sesuai SOP, menerapkan prinsip-prinsip kesehatan dan keselamatan kerja bagi petugas jaga.
